Housing Stabilization Services (HSS)
At Rose Care LLC, we recognize the challenges of navigating the housing landscape. That’s why we provide tailored support to help individuals find and sustain stable housing. Through our comprehensive approach, we assist clients in addressing obstacles such as securing employment, applying for benefits, and finding suitable housing. Our Housing Stabilization Services (HSS) Medical Assistance Program is designed to deliver the essential support needed for each client’s unique path to stability and independence.
Housing Transition Services
At Rose Care LLC, we specialize in helping clients navigate the journey to secure and settle into their ideal home.
● Developing customized plans to facilitate smooth housing transitions.
● Assisting with applications for benefits and programs to make housing more affordable and accessible.
● Guiding clients through the housing search, application, and evaluation processes.
● Providing reliable transportation for housing-related meetings and appointments.
● Educating clients about housing options, requirements, and processes to ensure informed decisions.
● Coordinating resources for deposits, moving logistics, and establishing safe, stable living environments.
Housing Sustaining Services

● Providing transportation for housing-related needs and appointments.
● Assisting with benefit applications to support long-term housing stability.

Housing Consultation Services
Housing Consultation Services
Rose Care LLC offers Housing-Focused Person-Centered Plans to assist individuals without case management services in connecting with providers for housing transition and sustaining services. These services include:
● Helping individuals gather necessary documentation to determine eligibility for Housing Stabilization Services.
● Creating a Housing-Focused Person-Centered Plan tailored to assessment results.
● Identifying strengths, needs, and housing preferences, including cultural considerations.
● Supporting informed decision-making when selecting housing transition or sustaining service providers.
● Offering resources to address non-housing goals identified during the person-centered planning process.
● Coordinating with other service providers involved in the individual’s care.
● Educating clients about their privacy rights and appeal processes.
● Annually reviewing and updating the person-centered plan to align with housing goals.
Who is Eligible?
* Be 18 years or older
* Have a disability or disabling condition
* Requires assistance with at least one of the following areas due to a disability or disabling condition:
– Communication
– Mobility
– Decision making
– Managing challenging behaviors
Have housing instability, such as:
– Homeless
– Transition from an institution or licensed registered setting
– At-risk for homelessness
– At-risk of institutionalization
